THIS ROLE IS LOCATED ALONG THE OREGON COAST We are hiring IMMEDIATELY for a Clinical Counselor to join one of our community mental health providers!
Summary:
Participate in a multi-disciplinary team providing program specific services to individuals and/or families served.
Ensure that the Individual Service Plan meets client needs.
Develop and maintain an on-going supportive relationship with the individual(s) served, as well as community partners also involved in the client's service plan.
Essential Functions/Major Responsibilities Maintain regular contact with the individual, family (or identified support system), and representatives of other systems in which the client is involved.
Provide case management services as needed.
Must be able to identify when the client is in crisis, follow crisis response guidelines, and inform relevant contact for necessary intervention.
Maintain the individual's case file, gather and transmit fidelity measures, and assure that they are submitted according to protocol.
Collaborate with the multi-disciplinary team in developing and implementing a transition plan from one service level to another.
Minimum Education Level Bachelors Specifics:
In behavioral science field.
Minimum Experience Level Two- Three years Specifics:
clinical/treatment experience in behavioral health field.
Certifications:
Specifics:
Classified as a Qualified Mental Health Associate.
